Job summary

Discovery At Home LLC in Naples, FL is seeking a PRN on-call nurse to provide comprehensive home health care. Responsibilities include assessments, care planning, medication administration, documentation, and collaboration with the care team to ensure high-quality outcomes.

The role offers on-call visits with variable compensation per visit: Start of Care 125.00, Recertification 80.00, Routine Visit 55.00, with billing and compliance support.

Qualifications

• Must be licensed or registered by the applicable state without restrictions.
• Bachelor's degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited program.
• One year of home health experience required
• Knowledge of Medicare and Medicaid guidelines
• Valid driver's license and reliable transportation
• Current CPR certification
• Excellent verbal and written communication skills
• Basic computer skills; ability to use Microsoft Windows
• Florida Level 2 background screening per AHCA requirements (fingerprinting)

Responsibilities

• Provide comprehensive nursing care to patients, adhering to professional nursing standards, policies, and procedures.
• Assess patient needs, develop, and implement individualized care plans to ensure optimal patient outcomes.
• Administer medications, treatments, and therapies as prescribed by healthcare providers.
• Monitor and document patient progress, including vital signs, symptoms, and changes in medical conditions.
• Collaborate with interdisciplinary healthcare team members to coordinate and provide continuity of care.
• Educate patients and their families about treatment plans, medications, and self-care techniques.
• Respond to emergency situations and perform life-saving interventions promptly and accurately.
• Maintain accurate and up-to-date medical records of patients’ health history, treatment plans, and progress.
• Adhere to infection control protocols and ensure a safe and healthy environment for patients and staff.
• Stay current with advances in nursing practices and attend training programs and continuing education to enhance professional development.

Background Screening Requirement: Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
