Job summary

Discovery at Home, part of the Discovery Senior Living family, is seeking a PRN Registered Nurse to provide comprehensive home health care in the Englewood, Port Charlotte and Punta Gorda market. You will assess patients, implement plans, administer treatments, and educate families while collaborating with the care team.

The role requires active nursing licensure, BSN/ADN, CPR certification, and at least one year in home health, with reliable transportation and strong communication skills.

Qualifications

• Must be licensed or registered by the applicable state without restrictions.
• Bachelor’s degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.
• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines.
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Responsibilities

• Provide comprehensive nursing care to patients, adhering to professional nursing standards, policies, and procedures.
• Assess patient needs, develop, and implement individualized care plans to ensure optimal patient outcomes.
• Administer medications, treatments, and therapies as prescribed by healthcare providers.
• Monitor and document patient progress, including vital signs, symptoms, and changes in medical conditions.
• Collaborate with interdisciplinary healthcare team members to coordinate and provide continuity of care.
• Educate patients and their families about treatment plans, medications, and self-care techniques.
• Respond to emergency situations and perform life-saving interventions promptly and accurately.
• Maintain accurate and up-to-date medical records of patients' health history, treatment plans, and progress.
• Adhere to infection control protocols and ensure a safe and healthy environment for patients and staff.
• Stay current with advances in nursing practices and attend training programs and continuing education to enhance professional development.
