Job summary

Frontpoint Health, LLC is seeking a Home Health Licensed Vocational Nurse (LVN) for PRN/per visit assignments in Lancaster, TX and Waxahachie, TX. The LVN provides in-home skilled nursing and coordinates care with the interdisciplinary team.

Responsibilities include administering medications, assisting with daily living activities, documenting care weekly, and participating in in-service trainings. Valid LVN license, CPR, TB, and auto insurance are required, with 1-3 years of LVN experience.

Qualifications

• Active state LVN license (NLC accepted).
• 1-3 years LVN experience.
• Proof of CPR, TB, and auto insurance.

Responsibilities

• Performs skilled nursing services under the supervision of a staff LVN.
• Observes, records, and reports changes in patient condition to the supervisor.
• Administers prescribed medications and treatments per state regulations.
• Assists physician or staff LVN in specialized procedures.
• Assists patients with activities of daily living and encourages self-care.
• Prepares clinical/progress notes and updates the clinical record weekly.
• Reports pertinent changes in patient condition to the physician and staff.
• Performs complex wound care if documented competency exists.
• Receives and follows physician orders within LVN practice guidelines.
• Participates in in-service educational meetings (at least twice yearly).
• Participates in nursing reports and staff conferences.
• Participates in QAPI plan and process.
