Job summary

SHC WVUHS Home Care in the United States is seeking a Licensed Practical Nurse to provide direct and indirect nursing care in patients' homes under RN supervision. The LPN will implement and evaluate the nursing plan with the RN, ensuring safety, privacy, and high-quality care in the home setting.

The role requires maintaining assessments, documenting interventions, and communicating changes to the RN and care team. Travel to patient homes is expected as part of home health services.

Qualifications

• Current Licensed Practical Nurse license in the state where services will be provided.
• Valid driver’s license.
• CPR certification within 30 days of hire.
• One year of Home Care Nursing experience preferred.

Responsibilities

• Performs and monitors basic patient care per policies and procedures.
• Uses nursing process: assessment, planning, implementing, and evaluating.
• Provides a safe and comfortable home environment for the patient.
• Ensures patient privacy and dignity during care.
• Communicates patient care needs to the primary RN and care team.
• Maintains patient confidentiality and documents care provided.
• Performs CPR when required and documents actions taken.
• Follows care plans and updates as directed by physician orders and RN.
