Job summary

Mountaineer Home Care, a part of LHCG Group and UnitedHealth Group's family of companies, seeks an Licensed Practical Nurse in Home Health to provide quality patient care in the patient's home.

Responsibilities include delivering clinical services per the plan of care, thorough documentation in the EMR, and active care coordination with the health team. CPR, home care experience, and strong communication are preferred.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance or access to public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and infection control procedures.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, communicates with patient, family, physician and care team.
• Works to accommodate diverse patient needs and flexible hours as required.
