Job summary

SHC WVUHS Home Care in Martinsburg, WV, is seeking an LPN to provide direct and indirect nursing care in patients’ homes under RN supervision. You will perform assessments, administer medications, and coordinate with the care team to ensure quality, compassionate care.

Qualifications

• Current LPN license in the state where services will be provided.
• Valid driver’s license.
• CPR certification within 30 days of hire.
• One year of home care experience preferred.

Responsibilities

• Performs and monitors basic patient care per policies and procedures.
• Uses nursing process: assessment, planning, implementing and evaluating to meet the patient’s needs.
• Provides a safe and comfortable home environment for the patient.
• Communicates patient care problems or potential problems to the RN and team to assure continuity of care.
• Maintains patient confidentiality according to policies and procedures.
• Documents nursing assessments and interventions with RN supervision.
• Administers medications by order of a physician and with RN oversight.
