Job summary

Elara Caring is seeking a Home Health Licensed Practical Nurse (LPN) for PRN assignments from Beverly to Peabody. Bring compassionate nursing care to patients in their homes, with one-on-one support and the flexibility of PRN shifts.

You will follow plans of care, educate patients and caregivers, document accurately, and collaborate with an experienced clinical team to improve outcomes and reduce unnecessary hospital visits. Massachusetts licensure and at least one year in home care are required.

Qualifications

• Massachusetts LPN license valid.
• Graduate of an accredited nursing program.
• At least one year of home care experience.
• Valid driver's license and reliable transportation.
• Ability to travel throughout Malden, Lowell, and surrounding communities.

Responsibilities

• Provide skilled nursing care under the direction of an RN and physician.
• Follow individualized plans of care and physician orders.
• Monitor patients and report changes to the RN Case Manager.
• Educate patients and caregivers on medications, treatments, and self-care.
• Complete accurate and timely clinical documentation.
• Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits.
