Job summary

HCA Healthcare’s Methodist Healthcare at Home is seeking a PRN Home Health Aide to provide direct patient care under a licensed nurse. The role involves assisting with activities of daily living, documenting care, and maintaining patient dignity in the home setting.

You will support a delivery-focused care plan, communicate changes in patient status to nursing staff, and work to maintain confidentiality and high-quality care for patients in their homes.

Qualifications

• DL and CNA license required.
• HHA certification required.

Responsibilities

• Assist patients with activities of daily living (bathing, dressing, grooming, toileting, transferring, meals).
• Document care delivery in the patient’s clinical record.
• Implement documented patient care and maintain confidentiality at all times.
• Maintain clear lines of authority and communicate effectively with the team to support patients.
• Promote positive relationships with patients, families, and co-workers.
• Identify changes in patient status and communicate with the nurse promptly.

Job Summary And Qualifications

Under the supervision of a professional nurse, provides direct care to patients. Assists in meeting the patient’s personal needs while treating the patient with respect and dignity.

What you will do in this role:

• Assists patients with the activities of daily living, including bathing, dressing, grooming, toileting, transferring, and getting to and from activities and meals according to the patient care/service plan.
• Documents the delivery of quality care in the patient’s clinical record.
• Implements patient care that is appropriately documented; maintains confidentiality at all times.
• Maintains consistent and concise lines of authority and responsibility with effective communication that
• promotes a positive working relationship with patients, families, and co-workers.
• Identifies changes in patient status or needs and communicates to the nurse timely.

What qualifications you will need:

• (DL) Driver License
• CNA License
• HHA Certification
