Haven Home Health & Hospice in Springfield, MO is seeking a Licensed Practical Nurse to provide in-home skilled nursing services, supporting the established plan of care and working closely with the RN and interdisciplinary team.

You will assess, document, and monitor patient care to ensure compassionate, clinically sound care in the home setting. Comprehensive benefits, flexible scheduling, and company vehicle (where available) are offered.

Formación

• Current Missouri LPN license.
• At least one year of LPN experience.
• Home health experience preferred.
• Strong clinical documentation abilities.
• Ability to collaborate with RN and clinicians.

Responsabilidades

• Recognize and report changes in patient condition to the RN supervisor.
• Complete timely, accurate clinical documentation.
• Support and carry out the patient’s plan of care alongside the RN.
• Collaborate with therapists and other clinicians to ensure continuity of care.

Job Summary

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care. You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team. Your role is essential in ensuring patients receive consistent, compassionate, and clinically sound care in the place they call home.

• Salary Range: $54,600 to $72,800 annually, based on experience

Duties and Responsibilities
