Job summary

Addus HomeCare in Missouri West Plains is seeking a licensed practical nurse (LPN) to provide nursing support to clients in their homes. The LPN conducts assessments and basic nursing procedures under RN supervision.

We emphasize quality patient care and independence, offering a FLEXIBLE SCHEDULE with no weekends and no on‑call requirements. Hours are self‑scheduled during weekdays.

Qualifications

• Graduation from an accredited school of nursing and current LPN licensure in the state of practice.
• New nurses welcome, will train!
• Ability to make home visits and provide nursing care in the home environment.

Responsibilities

• Medication set-ups and reviews
• Skin assessments and general health evaluations
• Client education and documentation
• Enable patients to stay independent while they remain in their own homes
• Perform routine supervisory visits
• Follow up for unskilled services
• Maintain confidentiality
• Maintain regular, predictable attendance and flexibility to meet agency needs
• Follow Medicaid and HIPAA regulations
• Abide by all regulations, policies, procedures and standards
• Performs other duties as assigned

We are seeking a LPN to provide nursing support to clients within our agency. This role is critical to meeting current care plan requirements, completing home visits and ensuring our clients can safely remain with our services. The Licensed practical nurse performs assessments and simple medical nursing procedures in client's homes, under the guidance of the RN Supervisor, in accordance with physician's orders.

This home care opportunity has No weekends and No On-Call schedule!

Hours: self-scheduled during the week. Monday through Friday. No weekends!

Service Areas: Mt. Grove, Willow Springs, Cabool and West Plains.

• Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.

• Abide by all regulations, policies, procedures and standards.
• Performs other duties as assigned.

• Home Care experience preferred but not mandatory
• Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
• Ability to complete necessary documentation appropriately, legibly, and timely
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
