Qualifications

• Must have a full, active license as a Registered Nurse.
• Minimum of two years related work experience in a geriatric healthcare setting.
• Flexibility for assessments and scheduling trainings.
• Demonstrates effective verbal and written communication skills.
• Ability to work independently.
• Demonstrates initiative, problem solving, adaptability and flexibility.
• Clinical skills: documentation of care, medication administration, infection control, patient education, physical assessment, vital signs, care coordination.

Responsibilities

• Perform basic nursing procedures including administration of medications and fluids and other nursing duties.
• Train caregiving staff on RN delegated care.
• Assess patient needs, develop and implement an individualized plan of care.
• Observe, report and document changes in patient condition with nursing interventions.
• Complete required reports and client assessments.
