Job summary

Suvida Healthcare seeks a PRN Float Provider (APNP or Physician) to drive, support, diagnose, and care for patients across multiple centers in a value-based care model. The provider will lead their Neighborhood Center care team and collaborate with a multidisciplinary group to deliver 5-Star patient care.

The role requires 3+ years experience (geriatrics preferred), active medical license in TX or AZ where centers are located, NP/PA national certification, and DEA license.

Qualifications

  • MD/DO required and licensed to practice in the work location state.
  • Active, unrestricted medical license in TX or AZ depending on center.
  • NP or PA with national certification.
  • Board certification or eligibility in Family or Internal Medicine.
  • Current DEA license.

Responsibilities

  • Cares for patients and focuses on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention.
  • Manages patients in a multidisciplinary value-based care model.
  • Serves as the leader for their respective care team within the Neighborhood Center.
  • Collaborates with the multidisciplinary care team.
  • Provides 5-Star care and high level of customer service.
  • Keeps medical history and medical records.
  • Refers patients to specialists as needed.
  • Attends daily morning huddle to discuss patients and close care gaps.
  • Attends weekly huddle to review and manage high-risk patients.
  • Provides coverage across multiple centers as assigned.
  • Adapts quickly to new teams, workflows, and patient populations.
  • Supports centers during provider absences, openings, or high patient demand.
  • Supports care team alignment during coverage assignments.

What You’ll Do

Position Summary

The PRN Float Provider (APNP or Physician) serves as a health-care professional who drives, supports, diagnoses, and cares for a variety of health-related problems for Suvida Healthcare’s patients. The provider will manage these patients in a multidisciplinary value-based care model across multiple centers. The physician work assignments involve low-emerging risk complex patients to high complexity patients where the analysis of situations or data requires an in-depth evaluation of variable factors. The clinician will collaborate with a multidisciplinary care team and will serve as the leader of their respective care team within their Neighborhood Center.Essential responsibilities consist of but are not all inclusive:

  • Reports to the Neighborhood Center Medical Director

What You’ll Bring

Knowledge, Skills, and Abilities

  • 3+ years minimum experience, preferably in geriatrics
  • Current on all required immunizations
  • A working understanding and support of a managed care, full risk, value-based care model preferred
  • Strong leadership, customer service, and spoken/written communication skills
  • Must have the ability to demonstrate self-confidence and positive attitude toward self and others
  • Excellent team player and communication skills required
  • Ability to quickly build trust with new teams and patients
  • Proficiency in Microsoft office platforms
  • Bilingual (English and Spanish) Preferred

Education, Experience, Licensure, or Certification Requirements

  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) in the state of the work location
  • Active, unrestricted medical license in Texas (for Tx. Centers) or Arizona (for Az. Centers)
  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) with National Certification
  • Board certification or eligibility in Family or Internal Medicine
  • Current DEA license
