Job summary

Chelsea Retirement Community is seeking a PRN Direct Care Supervisor (CNA/MA) to guide and support caregivers, ensuring compassionate, resident-focused care. The role is per diem with a flexible schedule, including weekend shifts, and offers hourly compensation.

You will mentor staff, coordinate patient care, supervise daily living activities per ISP, and help maintain safety and quality in memory care. Strong CNA/MA background is required and leadership is a plus.

Qualifications

• A high school diploma or equivalent is a must.
• Got training as a Medical Assistant, or Certified Nurse Aide (CNA)? Perfect!
• If you have experience in medical assisting, EMS, or caregiving—awesome! We’d love to hear about it.
• Bonus points for leadership or supervisory experience!
• You should know your way around medical terminology, medications, and body structure.
• Hands-on experience with injections and wound care? Even better (don’t worry, an RN will evaluate your skills).
• CPR certified? Great! If not, no worries—you’ll need to get it within 90 days.
• Need to lift up to 35 lbs.? You got this! Heavier tasks? We’ve got the right equipment and team support

Responsibilities

• Coach and mentor caregivers with hands-on training and feedback.
• Monitor incident reports and ensure policies and procedures are followed.
• Provide hands-on care including bathing, grooming, dressing, and toileting.
• Support mealtimes and ensure residents stay hydrated.
• Monitor changes in conditions and report to leadership ASAP.
• Demonstrate respect and professionalism while maintaining confidentiality.

Compensation: $20.00 - $25.00 / hour

Description

Schedule: Flexible-Per Diem |Work a minimum of 4 shifts per month, based on needs of the department. Two (2) shifts must be on a weekend.

Shift Premiums: 2:30 pm - 10:30 pm $ 1.90 | 10:31 pm - 7:00 am $ 1.35 | Weekend (Fri 10:31 pm - Sun 11:00 pm) $ 1.00

What You’ll Do:

Join our team as a Direct Care Supervisor, where you'll guide and support caregivers, ensuring our residents receive the compassionate care they deserve. In this role, you’ll be a key resource for your team, providing mentorship, problem-solving, and fostering a positive, team-oriented environment.

Key Responsibilities

• Lead and Support - Supervise caregivers in performing daily living activities based on individualized service plans (ISPs).
• Maintain Smooth Operations - Ensure proper staffing levels and collaborate with providers.
• Coordinate Medication and Care - Process medication orders and oversee resident assessments during intake and admission.
• Coach and Mentor - Provide hands-on training, feedback, and guidance to caregivers, helping them grow and excel.
• Monitor Safety and Compliance - Review incident reports and ensure policies and procedures are followed.
• Provide Hands-On Care - Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
• Support Mealtime - Help residents enjoy their meals and stay hydrated for good health.
• Monitor with Watchful Eyes - Report any changes in conditions to the leadership team ASAP.
• Demonstrate Respect & Professionalism - Maintain confidentiality while delivering top-notch care.
