Paragon Home Care in Northern Virginia is seeking an experienced Registered Nurse (RN) to conduct in-home client assessments and develop individualized plans of care for our caregiving team. This role is not hands-on or bedside nursing; visits focus on assessing needs, home environment, and safety, typically about 60 minutes plus documentation.

Responsibilities include conducting comprehensive assessments, developing and updating care plans, and documenting in AxisCare with training provided.

Qualifications

• Active RN license in Virginia.
• Experience assessing older adults in home care or home health.
• Ability to work independently with minimal supervision.

• Document assessments and care plans in AxisCare (training provided).
• Communicate relevant client needs or changes with the office team.

About the Role

Paragon Home Care is a non-medical home care agency serving older adults throughout Northern Virginia. We are seeking an experienced Registered Nurse (RN) to conduct in-home client assessments and develop individualized plans of care for our caregiving team. This is not a hands-on or bedside nursing position. Visits primarily involve assessing the client's needs, home environment, and safety. Most assessments take approximately 60 minutes, plus documentation.

Schedule & Pay

• Set your own schedule and choose which assessments you accept.
• Visits are typically completed on weekdays during business hours.
• Flat fee per completed assessment.
• Independent Contractor (1099) position.

Requirements

• Active Registered Nurse (RN) license in the state of Virginia
• Prior experience assessing older adults: home health, home care, or community nursing preferred
• Strong clinical assessment and critical thinking skills
• Excellent documentation skills
• Reliable transportation, valid driver's license, current auto insurance
• Ability to work independently with minimal supervision
• Strong interpersonal and communication skills with patients, families, and care teams
• Able to clear a Virginia criminal background check (required by state law for home care organizations)
• Must provide TB screening documentation
