Job summary

Responsibilities include disease management coordination, outreach for high‑risk patients, and facilitating transitions of care while partnering with primary and specialty care teams.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification via AHA or Red Cross.
• Minimum one year recent RN experience.

Responsibilities

• Collaborate with a multidisciplinary care team to develop goals and maximize outcomes.
• Provide care coordination and disease management support for high‑risk patients.
• Identify patients requiring ongoing coordination and conduct targeted outreach.
• Outline involvement needed by care team and primary care.
• Use assessment tools to identify care needs requiring coordination.
• Utilize registries and lists to manage patient populations and plan care.
• Conduct comprehensive clinical assessments across medical, behavioral and social needs.
• Educate patients and families about care coordination and monitor progress.
• Serve as liaison and advocate for patients and families.
• Assist in transitions of care and ensure clear communication.
• Identify barriers and connect to community resources.
