Job Summary:

The purpose of the Certified Nursing Assistant (CNA) position is to provide routine daily nursing care and services to assigned Patients according to the care plan.

Qualifications:

• Completion of CNA or Certified Home Health Aide Program
• At least 1 year of Home Health Aide or Nursing Assistant experience in a hospital, nursing home, or home health agency
• Reliable transportation with valid auto liability insurance
• Good communication skills to relate professionally to patients, caregivers, and the clinical team
• Ability to read, write, and speak English
• Interest in the welfare of the ill and elderly
• Current CPR and Hepatitis profiles
• Successful completion of Aide Competency Evaluation Skills Checklist and Skills Test

Essential Functions:

• Provide and document personal care in accordance with policies, procedures, and regulations
• Support patient independence
• Report patient status to charge nurse at shift start and end
• Document observations and contribute knowledge of patient conditions
• Maintain a safe and clean environment
• Follow procedures for safe lifting and transferring patients
• Ensure equipment and mobile devices are charged, clean, and functional
• Attend annual in-service training to maintain certification
• Perform incidental housekeeping as needed
• Function as a team leader/role model
• Enforce abuse prevention policies
• Participate in weekly in-service meetings
• Ensure patient safety at all times
• Perform other duties as assigned
