Qualifications

Responsibilities

• Evaluate physician medication orders for appropriateness.
• Maintain patient/resident database with medical history and medication.
• Order and evaluate laboratory tests for high-risk conditions.
• Document communication with healthcare team and patients.
• Implement policies for safe drug procurement and disposal.
• Inform Pharmacy Manager of medication-related errors.
