Continuum Rx is seeking a PRN Pharmacist to provide safe, accurate pharmacy services for infusion and specialty therapies. This role collaborates with nurses and providers to optimize medication therapy and ensure safe transitions from hospital to home.

Responsibilities include reviewing orders, assessing regimens, monitoring pharmacokinetics, and advising on antimicrobial therapy. Strong clinical judgment and communication are essential for success.

Competenze

Doctor of Pharmacy (PharmD) or equivalent.
Strong clinical assessment and medication-management skills.
Excellent written and verbal communication skills.

Mansioni

Review home infusion medication orders for accuracy, appropriateness and safety.
Assess patient-specific regimens including dose, route, frequency and interactions.
Monitor pharmacokinetic dosing for vancomycin and aminoglycosides.
Review labs and make recommendations on therapy.
Collaborate with physicians, nurses and other healthcare professionals.
Provide education to patients, caregivers and staff as appropriate.
Oversee preparation of compounded sterile products per procedures.
Maintain accurate pharmacy documentation.
Communicate changes and monitoring requirements to care teams.
Assist with after-hours or urgent clinical needs as part of PRN team.
