Qualifications

• Licensure as RN, SW, OT, PT, ST, or Rehabilitation Counselor required.
• 2 years of rehabilitation experience preferred.
• Ability to independently complete assessments.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Monitor patient experience and compliance with regulations.
• Facilitate interdisciplinary team conferences.
