Qualifications

• Two years’ relevant experience in case management.
• Knowledge of community resources to meet post-discharge needs.
• Ability to maintain confidentiality and communicate effectively.

Responsibilities

• Coordinate with physicians and the multidisciplinary team to develop a care plan.
• Monitor patient progress through assessments and records to ensure positive outcomes.
• Identify and resolve barriers hindering patient care and participate in discharge planning.
• Ensure compliance with accreditation standards and post-discharge processes.
• Communicate with payors for timely authorizations and updates to the clinical team.
