Job summary

ClearSky Health is seeking a Case Manager to join our multidisciplinary team at our Eau Claire location. You will coordinate care with physicians, nurses, therapists, patients, and families to develop plans of care and remove barriers to treatment, supporting successful patient outcomes.

The role emphasizes discharge planning, documentation, and compliance with accreditation and state standards. Full-time, Monday–Friday, 8:00 AM–5:00 PM.

Qualifications

• Bachelor's degree or active clinical licensure; acute care setting preferred.
• Strong communication and documentation skills.
• Ability to coordinate with physicians and multidisciplinary teams.
• Knowledge of discharge planning and utilization review.

Responsibilities

• Coordinate care plans with physicians and the multidisciplinary team.
• Monitor patient progress and remove barriers to treatment.
• Participate in discharge planning and coordinate post-discharge needs.
• Maintain patient records and ensure regulatory compliance.
