Qualifications

• Master’s in Social Work/Care Management required
• 2+ years care management experience preferred
• 2+ social work experience required

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients.
• Assesses patients and families holistically for discharge planning needs in inpatient, observation and emergency departments.
• Develops discharge plans with contingency plans to ensure timely care coordination.
• Participates in multi-disciplinary rounds to review patient status and discharge plans.
