Job summary

Holy Cross Hospital – Jordan Valley in Utah seeks a Behavioral Health Social Worker for a PRN role to participate in patient assessment, counseling, support, and resource linkage across acute care, outpatient, and community settings.

Responsibilities include collaborating with physicians and staff, leading groups, and educating patients and families about illness and treatment options. Requires MSW or equivalent, licensure, and BLS; PRN offers a 15% pay differential.

Qualifications

• Master's degree in Social Work, Counseling or related required.
• One year of related clinical experience required.
• Current State License as CSW/LSW or LPC required.
• Current Basic Life Support from the American Heart Association required.

Responsibilities

• Assess psychosocial history, cultural and spiritual needs through observation, interviews and clinical data.
• Provide counseling to patients and families and organize support groups.
• Coordinate with physicians and medical staff to aid treatment decisions and resource linkage.
• Advocate for patients and families, addressing ethical dilemmas and unmet needs.
• Educate patients and families to increase knowledge of illness, treatment options and appropriate care levels.
