Provides quality care to patients of all ages through the proper utilization of assessment, planning, implementation and instruction. Carries out all procedures in accordance with established state and national standards, policies, protocols and guidelines.

• Graduate (e.g., bachelors, master, or doctorate) from an accredited program which meets the licensure requirements in the state of practice (Required).
