Qualifications

• WV: Temporary Social Worker License OR Licensed Graduate Social Worker OR Licensed Professional Counselor Temporary Permit/Provisional LPC.
• OH: Licensed Social Worker per Ohio Board.
• MD: Licensed Master Social Worker OR Licensed Graduate Professional Counselor.
• PA: Licensed Social Worker per PA Bureau of Professional and Occupational Affairs.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Create comprehensive psychosocial treatment plans using clinical social work interventions.
• Screen, diagnose, and treat mental health and/or substance use problems in patients and families.
• Assist with screening, identification, diagnosis, management and treatment of abuse, neglect, domestic violence, rape, etc.
• Maintain knowledge of medical/legal issues affecting patient care.
• Provide education to patients and families about diagnosis, illness and treatment.
• Participate in multi-disciplinary health care teams.
• Maintain liaison with community agencies and resources.
• Access and provide for patient care needs using hospital resources.
• Document assessment, plan, interactions, and interventions per guidelines.
• Attend staff, committee and department meetings.
• Keep current with social work and health care developments.
