Crossover Health is seeking an Optometrist to deliver primary vision care as part of a patient-centered medical home. You will conduct eye exams, diagnose vision problems, and create treatment plans while collaborating with a multidisciplinary team.
The role requires a minimum of 3 years in patient-centered optometry, state licensure, and a current BLS certificate. 401K eligibility is offered, with flexible alignment to the surrounding community and care network.

The base pay range for this position is $68.86 to $90 per hour. Pay range may vary depending on work location, applicable knowledge, skills, and experience.
This position will be eligible for 401K.
