We’re seeking an experienced and compassionate Per Diem Registered Nurse (RN) to complete client assessments, care plans, and clinical oversight visits for our private-pay home care clients. Position Summary

Offering $100/visit. The Per Diem RN Assessment Nurse provides skilled, person-centered nursing assessments, develops individualized care plans, and ensures compliance with Minnesota Department of Health (MDH) home care regulations. The RN also supports field caregivers, performs supervisory visits, and promotes safe, quality care in the client’s home setting. This role is perfect for nurses with strong clinical judgment, home care experience, and a passion for working independently in the community.

Preferred

• Experience as a Home Health RN, Case Manager, or Field/Assessment Nurse
• Prior experience with Alzheimer’s, dementia, or chronic illness care
