Job summary

Legacy Concierge is seeking a dedicated Private Duty Registered Nurse I to deliver exceptional 1:1 home care to patients in California. You will work under clinical supervision with structured onboarding and a clear clinical career pathway.

Responsibilities include hands-on nursing, assessments, medication administration, and documentation while ensuring patient-centered care in a private home setting.

Qualifications

• Active California RN license in good standing.
• Graduate of ADN or BSN nursing program.
• Ability to work independently in a 1:1 home care setting.
• Strong communication and documentation skills.
• Current BLS/CPR certification.

Responsibilities

• Deliver 1:1 skilled nursing care in the patient’s home.
• Perform head-to-toe assessments including neurological, respiratory, cardiovascular, skin, and MSK status.
• Monitor and document vital signs during shifts.
• Administer medications per plan of care (e.g., insulin, BP meds, oxygen).
• Support ADLs/IADLs and provide companion care.
• Perform G/J-tube feeding and medication administration as ordered.
• Provide condition-specific care per the plan of care.
• Perform basic wound care and dressing changes.
• Complete daily clinical notes and maintain HIPAA compliance.
• Escalate changes to the RN Care Ambassador and communicate with providers as directed.
• Attend training and onboarding including shadow shifts.
