Legacy Concierge is seeking a dedicated Private Duty Registered Nurse to deliver 1:1 in-home nursing care with mentorship and a defined clinical career path. You will conduct comprehensive assessments, monitor vitals, and administer prescribed treatments in a private home setting.

The role emphasizes patient-centered care, strict HIPAA compliance, and seamless communication with care teams while supporting ADLs and ensuring safety during 12-hour shifts.

Qualifications

• Active California RN license in good standing.

Responsibilities

• Deliver 1:1 skilled nursing care in the patient's home.
• Perform comprehensive head-to-toe assessments.
• Monitor and document vital signs over the shift.
• Administer medications per care plan (insulin, BP meds, oxygen, etc.).
• Support ADLs and IADLs.
• Perform G/J-tube feeding as ordered.
• Provide condition-specific care per plan of care.
• Perform basic wound care and dressing changes.
• Complete daily clinical notes in the EMR.
