Job summary

Legacy Concierge is seeking a dedicated Private Duty Registered Nurse I to deliver 1:1 home care under mentorship, with a clear clinical career pathway. You will provide hands-on nursing, document care, and coordinate with the RN Care Ambassador to ensure continuity and high-quality patient outcomes.

The role emphasizes patient-centered care, structured onboarding, and the ability to work in a private home setting with 12-hour shifts.

Qualifications

• RN license active in California.
• Graduate of an accredited ADN or BSN nursing program.
• ADN graduates – minimum 2 years hospital experience; preferably ER/urgent care/telemetry.
• BSN graduates – minimum 8–12 months hospital or skilled nursing facility experience.
• Current BLS/CPR certification.
• Ability to work competently in a 1:1 private care environment.
• Strong communication, documentation, professionalism, and customer service skills.

Responsibilities

• Deliver 1:1 skilled nursing care in the patient's home.
• Perform head-to-toe assessments and monitor vital signs.
• Administer medications safely per plan of care.
• Support activities of daily living and IADLs.
• Perform G/J-tube feeding and medication administration as ordered.
• Provide condition-specific management and wound care as directed.
• Complete daily clinical notes and communicate changes to the RN Care Ambassador.
• Maintain confidentiality and HIPAA compliance.
