Job summary

Adara Home Health is seeking a dedicated Registered Nurse to provide 1:1 client care in Minnesota home settings. You will travel to client homes, collaborate with Case Manager, and deliver comprehensive nursing care including meds, treatments, and education.

The role emphasizes clinical judgement, strong communication, and the ability to work independently in diverse home environments. Part-time schedule with travel to service areas such as Mankato, Janesville, and surrounding communities.

Qualifications

• Active Minnesota RN license.
• Minimum 2 years in a healthcare setting.
• Home health or acute care experience preferred.
• Ability to make clinical judgments and plan care.
• Strong written and verbal communication skills.

Responsibilities

• Plan, provide, and evaluate nursing care per policy; consult Case Manager for care plan changes.
• Observe and evaluate client care using current health practices.
• Deliver total client care including meds, treatments, education.
• Coordinate with families and other health professionals to ensure continuity of care.
• Assess symptoms, interpret findings, and report changes promptly to Case Manager.
