Job summary

Adara Home Health is hiring a Clinical Admissions Nurse to ensure client care adheres to policies and maintains strong referral relationships. Travel to client homes for 1:1 care (1–3 hours per client) and report to the Clinical Supervisor. Coverage includes Hutchinson, MN, serving Watertown and surrounding areas.

The role requires MN RN licensure, 2+ years in health care, and strong nursing skills. Base salary range is $70,000–$95,000 per year with full-time benefits and on-call duties.

Qualifications

• RN license in Minnesota, active and in good standing.
• At least two years of healthcare facility or home care experience.
• Strong organizational and independent working skills.
• Ability to make appropriate clinical judgments.
• Excellent written and verbal communication.
• Basic computer skills; Word and Excel knowledge.
• Understanding of Medicare certification and OASIS documentation.
• Valid driver's license and reliable vehicle.

Responsibilities

• Performs initial and periodic client assessments and updates care plans.
• Provides total client care and education as ordered by the physician.
• Monitors symptoms, reports changes and ensures safety.
• Coordinates client records and care with family and resources.
• Maintains confidentiality and infection control compliance.
• Participates in after-hours on-call process.
