Legacy Concierge is seeking a dedicated Private Duty RN to provide one-to-one home care with mentorship and a clear clinical career path. The role emphasizes hands-on nursing, patient-centered care, and continuity in a private home setting.

Responsibilities include comprehensive assessments, vital signs monitoring, medication administration, and documentation. Onboarding is structured, with growth opportunities and full benefits for eligible hours.

Formación

• Active California RN license in good standing.
• Graduate of an accredited ADN or BSN nursing program.
• Current BLS/CPR certification.
• Ability to work competently in a 1:1 private home care environment.
• Strong communication, documentation, and customer service skills.

Responsabilidades

• Deliver 1:1 skilled nursing care in the patient’s home.
• Perform head‑to‑toe assessments and monitor vital signs.
• Administer medications per plan of care and manage ADLs/IADLs.
• Document daily clinical notes and maintain HIPAA compliance.
• Escalate changes to the RN Care Ambassador and coordinate with providers.
• Attend onboarding and competency evaluations.
• Support case management and maintain reliable scheduling.

Conocimientos

Active CA RN License

BSN or ADN degree

BLS/CPR Certification

Private duty/home health care

Educación
