Job summary

CONSUMER DIRECTED SERVICES INC. is seeking a Private Duty Registered Nurse (RN) to provide one-to-one skilled nursing care in patients' homes. You will perform assessments, administer medications, manage ventilators/traches, and document care in EMRs.

Responsibilities include wound care, patient and family education, and collaboration with an interdisciplinary care team to follow individualized care plans.

Qualifications

• Valid RN license and strong clinical judgment.
• Experience in complex home care (vent/trach and wound care preferred).
• Excellent communication and patient-first bedside manner.

• Administer medications safely and accurately
• Manage ventilators and trachs, ensuring airway and respiratory support
• Provide wound care and monitor healing and complications
• Deliver patient and family education to support home care
• Document care and clinical updates in electronic medical records

Private Duty Registered Nurse (RN)

Deliver clinically focused services including patient assessments, medication administration, ventilator and tracheostomy (trach) management, and wound care. Educate patients and families, and collaborate with an interdisciplinary care team to follow individualized care plans.
