Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• LA: Current LPN Licensure in LA without restrictions; 1+ year of experience as an LPN.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws.
• Documents patient visits thoroughly in the EMR and complies with regulatory requirements.
• Active participation in care coordination and effective communication with patient, family, physician, and team.
