Qualifications

• Active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification with hands-on component.
• TB skin test completed within the last 12 months.
• Six months hands-on nursing experience preferred.
• Compact licenses must be transferred to state of residence within 60 days.

Responsibilities

• Provide in-home LPN/LVN nursing care to patients.
• Collaborate with families and clinicians to achieve optimal patient outcomes.
• Document care using electronic systems and maintain compliance with regulatory requirements.
