Job summary

Interim HealthCare Inc. is seeking a Private Duty Nurse (PDN) Licensed Practical Nurse (LPN) to provide 1:1 in-home skilled nursing care.

You’ll support longer, consistent shifts and build relationships with clients and families, delivering comfort, safety and dignity at home. As part of our team, you’ll document patient conditions, administer medications, monitor vitals, and collaborate with RNs and care coordinators to ensure seamless care across shifts.

Qualifications

• Graduate of an accredited practical nursing program with current state LPN/LVN license.
• One year of clinical/home care experience preferred; preceptor program acceptable.
• Current CPR/AED/BLS/First Aid certification.

Responsibilities

• Provide 1:1 in-home skilled nursing care per care plan, supporting extended shifts.
• Partner with client/family and care team to maintain continuity of care.
• Assess, monitor, document patient condition and vital signs; report changes promptly.
• Administer medications and treatments with thorough documentation.
• Provide wound care and assist with personal care as outlined.
• Monitor for infection or complications and follow provider orders.
• Educate client and family on routines, equipment, and safety at home.
• Complete timely clinical notes and support shift handoff for continuity.
• Coordinate with supervising RN and office team to ensure outcomes.
