Cheer Health Care is actively hiring per diem Licensed Vocational Nurses (LVN) for private duty home care in La Jolla, San Diego North County. The role offers a competitive pay rate of $35.00–$50.00 per hour, with 12-hour day shifts on Monday, Wednesday, and Saturday.

You will deliver premium home-based nursing care to seniors and adults with complex needs, collaborate with physicians and family members, and ensure precise, timely documentation of patient progress.

Qualifications

Current, unrestricted California LVN license.
Graduation from accredited practical or vocational nursing program.
1 year of experience as a Licensed Vocational Nurse.
Home Health experience preferred.
Proof of Negative TB result.
Physical Screening Documentation within the last year.
Current BLS certification.
Valid California driver's License and insurance.
Cell phone with internet access.

Responsibilities

Advanced Clinical Management: administer medications, wound care, monitor chronic conditions.
Interdisciplinary Collaboration: coordinate with physicians, PTs, and families.
Family & Patient Empowerment: educate clients and caregivers on health updates and care plans.
Accurate Clinical Reporting: document client progress and care provided.
