Qualifications

• Active NYS LPN license required.
• Current CPR certification.
• Infection control certificate.
• Must be able to work assigned schedule.
• Home care or PDN experience preferred.
• Strong English communication skills; Spanish or Russian a plus.

Responsibilities

• Provide skilled nursing care in accordance with physician orders.
• Document patient condition and care provided.
• Follow patient care plans.
