Always Compassionate Health is seeking compassionate, reliable LPNs to provide one-on-one home nursing care in Nassau County, NY. You will administer medications, monitor patients, and collaborate with families to ensure safety and comfort.

Qualifications

• Active New York State LPN license is required.
• Current CPR/BLS certification.
• Minimum six months of home care experience is preferred.
• Strong clinical and communication skills are essential.
• Compassionate, dependable, and able to work independently.

Responsabilités

• Provide nursing care and administer prescribed medications and treatments.
• Perform G-tube/PEG care, tracheostomy care, and suctioning as needed.
• Monitor patients’ conditions and promptly report changes in health status.
• Provide ventilator support when required and appropriately trained.
• Educate patients and caregivers and maintain accurate clinical documentation.
