Qualifications

• LPN/LPT/LVN Degree from an accredited college with active license in state of practice.
• Valid CPR certification.
• 1+ years of nursing experience preferred.
• Home Health care experience preferred.

Responsibilities

• Provide 1:1 care to clients according to the plan of care
• Administer medications, feedings, oxygen, ostomy care per plan & assessments
• Document nursing actions and communicate with family, case manager, and physicians
• Coordinate or order supplies, medications, and feedings as needed
• Ensure patient needs are met per physician instructions
