Qualifications

• LPN/LPT/LVN degree with current license in state
• Valid driverf license with proof of insurance
• CPR certification
• 1+ years nursing experience preferred
• Home health care experience preferred

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Documenting nursing actions and communications with family, case manager, and physicians
• Coordinating supplies, medications, and feedings to ensure adequate on-hand
• Ensuring all patient needs are met per physician instructions
