Adoration Health is seeking a Private Duty LPN to join our Jackson, TN team. Provide dedicated 1:1 patient care in a home health setting, following each client’s plan of care with compassion and professionalism.

Qualifications

• LPN/LPT/LVN degree with current unrestricted state license.
• CPR certification required.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.
• Valid driver’s license and proof of auto insurance required.

Responsibilities

• Provide 1:1 care to clients following the plan of care.
• Administer medications, feeds, oxygen, ostomy care as needed per plan of care.
• Document nursing actions, communicate with family, case manager, and physicians.
• Coordinate supplies, medications, and feeds to ensure sufficiency.
• Ensure patient needs are met per physician instructions.
