Job summary

Adoration Health in Centerville, TN is seeking a Private Duty Licensed Practical Nurse to join our home-health team. This full-time role provides 1:1 patient care according to each plan of care, monitors conditions, and supports families with compassionate, skilled nursing.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice.
• Valid driver's license, acceptable driving record and proof of car insurance.
• CPR certification required.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Providing 1:1 care per client plan of care, maintaining dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed and assess results.
• Documenting nursing actions and communicating with family, case manager, and physicians; update goals.
• Ordering or coordinating supplies, medications, and feedings to ensure availability.
• Ensure patient needs are met per physician instructions.
