Health, Inc is seeking a Private Duty Nurse (LPN) to provide one-on-one skilled nursing care to patients in their homes across Pennsylvania. This role supports medically fragile pediatric and adult individuals whose stability and recovery depend on dedicated at-home care.

You will perform assessments, administer medications, manage wounds, and coordinate with families to implement individualized care plans, documenting findings clearly and promptly.

Qualifications

• Current LPN licensure.
• Proficiency with complex home care, including trach/vent and G-tube care.
• Strong clinical assessment, medication, and wound care skills.
• Excellent documentation and communication skills.
• Compassionate, patient-centered approach with empathy and professionalism.
