Job summary

At-Home-Care-of-Kentucky seeks licensed RNs or LPNs to provide skilled nursing care in patients' homes across Kentucky. You will follow service plans, administer medications, educate patients and families, and document progress notes in the agency system.

The role requires valid Kentucky nursing licensure, CPR certification, TB test, and ongoing clinical competency. Applicants should be able to work independently, manage on-call needs, maintain confidentiality, and adhere to infection control

Qualifications

• Valid Kentucky nursing license (LPN or RN) and current CPR certification.
• Current TB skin test or willingness to obtain.
• Competency in general nursing skills, techniques, and practice.
• Private duty experience helpful, but not required.

Responsibilities

• Provide skilled nursing care in the home per service plan and physician orders.
• Promote patient safety and reduce health care errors.
• Teach patients and families about health maintenance and rehabilitation.
• Prepare progress notes and submit visit verifications promptly.
• Communicate patient status to the care team and participate in on-call coverage.
