Job summary

At Home Healthcare in Union, KY is seeking a Home Health Nurse (LPN or RN) to provide in-home care under supervision of the Director of Patient Care Services. A valid Kentucky nursing license, CPR certification, and TB testing are required.

You will complete progress notes, follow care plans, coordinate with physicians and staff, and ensure patient safety in private homes. On-call duties may apply.

Qualifications

• Valid Kentucky nursing license (LPN or RN) required.
• Current CPR certification or willingness to obtain.
• Current TB skin test or willingness to obtain.
• Competency in general nursing skills, techniques, and practice.
• Private duty experience helpful, but not required.

Responsibilities

• Provide skilled nursing care in patients' homes per the service plan.
• Maintain patient safety and reduce risk of medical errors.
• Prepare clinical progress notes and follow the client service plan.
• Submit notes and visit verifications in a timely manner.
• Communicate patient needs to appropriate staff.
• Adhere to infection control and safety policies.
• Participate in the on-call schedule.
• Maintain confidentiality of all records and reports.
