Job summary

At-Home-Care-of-Kentucky is seeking a qualified home health nurse to provide skilled nursing care in patients’ homes across Kentucky. You will follow the service plan, physician orders, and agency policies while promoting patient safety and effective self-care.

You must hold a Kentucky nursing license (LPN or RN) with CPR and TB testing current or willing to obtain. Strong clinical skills, on-call availability, and strict infection control practices are expected.

Qualifications

• Valid Kentucky nursing license (LPN or RN).
• CPR certification or willingness to obtain.
• TB skin test or willingness to obtain.
• Competence in general nursing skills, techniques and practice.
• Ability to read, analyze and interpret medical orders.

Responsibilities

• Provides skilled nursing care in the home to patients in accordance with the service plan, physicians' orders and agency policies.
• Promotes patient safety and reduces medical/health care errors.
• Educates patients on health practices and self-care techniques.
• Prepares clinical progress notes and visit verifications.
• Communicates patient status to the care team and Director of Patient Care Services.
• Participates in on-call schedule to attend to care needs outside regular hours.
• Maintains confidentiality and adheres to infection control policies.
