Elite Medical at Home is seeking compassionate RN, LPN, and HHA professionals for private-duty home care across Boston. This role emphasizes 1:1 patient care in the home with flexible scheduling and no minimum shift commitment.

Opportunities include per-diem work, pediatric and adult cases, and care for high-acuity needs. Training and support are provided, with a focus on respectful, family-centered care in a private home setting.

Qualifikationen

• Current RN, LPN, or HHA credential/license/certification, as applicable.
• Must be authorized to work in Massachusetts.
• Current CPR certification preferred.
• Home care, private-duty, pediatric, adult, or complex care experience preferred.
• Nursing students are welcome to apply.
• Reliable, compassionate, and professional.
• Ability to work independently in a home-care setting.
• Comfortable working with patients and families in the home.

Aufgaben

• Provide 1:1 skilled nursing care in the patient’s home.
• Complete ongoing patient assessments and monitor changes in condition.
• Administer medications as ordered.
• Monitor vital signs and patient response to care.
• Assist with G-tube care, feeding support, and medication administration.
• Provide respiratory, tracheostomy, ventilator, ostomy, catheter, or wound care when applicable and within scope.
• Document care accurately and timely.
• Communicate with families, care teams, supervisors, and providers as needed.
• Follow the individualized plan of care.
• Support pediatric and/or adult patients with complex medical needs.
• Assist patients with activities of daily living.
• Support bathing, grooming, dressing, mobility, and personal care.
• Provide companionship and safety supervision.
• Assist with meal support and light care-related tasks.
• Observe and report changes in patient condition.
• Follow the care plan and communicate with the care team.
• Provide respectful, compassionate support in the home setting
