Qualifications

• 1 year of experience as a home health aide, CNA, LVN, or RN in a home health setting.
• Valid HHA certificate in the practicing state and CPR certification.
• Current driver’s license in good standing; reliable transportation and auto liability insurance.
• High school diploma or equivalent.
• Ability to lift up to 50 lbs; prolonged standing/walking.
• Willingness to travel locally and maintain a flexible schedule.
• Maintain infection control and universal precautions.

Responsibilities

• Assist clients with tasks within the Plan of Care (POC).
• Communicate effectively with administrative staff regarding client needs and changes.
• Provide care as outlined in the Care Plan and document the cares performed.
• Contribute to continuity of care and participate in Case conferences/Team Meetings.
• Apply infection control concepts to protect staff and patients.
• Participate in in-service training and maintain competency.
• Meet mandatory continuing education requirements.

General Summary

Assists clients, in their place of residence, with personal healthcare and health maintenance activities as assigned. Includes, but is not limited to, transferring, health maintenance activities, feeding, mobility, and exercises.

Patient Population

Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.

Essential Functions

• Assist clients with tasks within the scope of job description based on the Plan of Care (POC).
• Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
• Provide care as outlined in the Care Plan and accurately document the cares provided.
• Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
• Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
• Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.
• Meet mandatory continuing education requirements of the Agency/licensing board.

Requirements

Experience

• (1) year experience as a home health aide or certified nursing assistant or Licensed Vocational Nurse or Registered Nurse in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.

Education

• High School Graduate or equivalent

Current CPR certification.

• Current driver’s license in good standing. The employee is responsible for renewing their driver’s license before it expires to continue employment.
• Reliable transportation and auto liability insurance.
• On-going employment as an HHA always requires the maintenance of a valid HHA certificate in the state where the caregiver practices and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.

Physical Requirements

• Prolonged standing/walking is required.
• The ability to lift/transfer up to 50 lbs.
• Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
• Meet patient/family individualized psychosocial needs.
• Requires hand-eye coordination and manual dexterity.

Environmental/Working Conditions

• Works in patients’ homes in various conditions.
• Possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
• Some exposure to unpleasant weather; PRN emergency call.
• Ability to travel locally and work a flexible schedule.
